    While it isn't email survey software, you might
    want to check out http://www.zoomerang.com/index.zgi
    for web-based surveys. A web based cgi or Cold Fusion
    also does what you want.

    Hi colleagues,

    I am seeking advice on email survey software. I am looking for software that
    is commercially available at a reasonable cost. It needs to be able to
    launch from email, and have an option to ask different questions based on
    previous responses. It needs also to download even if only through excel to
    SPSS. Any recommendations?

    My organization has just purchased SurveyTracker software. It is so new to us that we have not yet had a chance to use it; I expect to conduct my first survey next week. We did a fair amount of research before choosing this product, though, and it came to us highly recommended. You can find more information at the vendor's website; I think (but I'm not certain) that it was the obvious http://www.surveytracker.com.
