The book, "Feedback to Managers, Volume II: A review and Comparison of Sixteen Multi-rater Feedback Instruments," by Ellen Van Velsor & Jean Brittain Leslie, published by the Center for Creative Leadership, 1991, has plenty of sample questions in it.
