    The book, "Feedback to Managers, Volume II: A review and Comparison of Sixteen Multi-rater Feedback Instruments," by Ellen Van Velsor & Jean Brittain Leslie, published by the Center for Creative Leadership, 1991, has plenty of sample questions in it.

    If you want to give your students an experience with 360 Feedback you might want to try either Employee Feedback: Revealing Leadership Blind Spots at http://perdue.ssu.edu/~fmshippe/leadersh.htm or An Interactive Exercise: Commitment, Tension, and Leadership Skills at http://perdue.ssu.edu/~fmshippe/comtenle.htm. The former is for small classes (approximately 15 or less) and the latter is for larger classes. I have used them with both undergraduate and graduate students as well as in management development seminars. I hope that you find these helpful.
