Thanks for the pointer to the Post-mortem questions.
Lessons Learned can be a valuable part of any group activity whether or not
the activity is called a project (although more gets done if every activity
is seen as a project).
Good list of questions but missing one -- "Are there other questions that
should be addressed?"
The other 90% of a post mortem is the way in which the participant's ideas
are melded and Lessons Learned derived. Simply "--hold a meeting and
discuss the team's responses to the questions. The result of this discussion
is often a list of "Lessons Learned."" is not sufficient. Most, and too,
often the Lessons Learned come out saying to do the opposite of what was
perceived as causing the problem. Doing the opposite is usually a futile
solution. Do you have more information on how to conduct such a meeting?
