Frank,
While far from "informal", you might try our 720Feedback(R) survey,
Coaching Practices(C). It's one of our 13 validated role-specific Task
Cycle(R) surveys. Tell us more about your applications and we might be
willing to approve limited informal usage. It is on our new .NET
platform, TruScore(TM), on which you can administer any kind of
assessment from any browser.

Subject: Informal Coaching Assessments
I am looking for an informal coaching assessment instrument for use in a
developing managerial skills class. The assessment can be either a
self-assessment or an assessment of one's superior. By informal
coaching, I am looking for an assessment of the everyday on the job use
of coaching, not that of formal coaching.
If you have one that you would be willing to share or know of one,
please let me know.
Thanks,
Frank
